The UK's MSK Pain Challenge

Musculoskeletal (MSK) conditions, like back and neck pain, are a significant burden. They account for around 30% of all GP appointments in the UK and are the leading cause of disability (Versus Arthritis, 2021), contributing to over 20% of all years lived with disability (IHME 2020). Despite this widespread impact, MSK pathways are often under-resourced, leading to delays in accessing crucial services like physiotherapy or rehabilitation. This often means patients are left with painkiller prescriptions or are referred for more invasive interventions, even when guidelines recommend conservative care as a first-line approach (NICE NG59, 2016).
The Assurance of Safety in Chiropractic Care
When you're exploring options for pain relief, safety is paramount. The good news is that chiropractic care, particularly spinal manipulation, has a strong safety profile. Major reviews of the topic conclude that while minor side effects can be common, serious adverse events are rare. For example, a large 2023 study of nearly one million chiropractic treatments found a severe adverse event rate of approximately 1 in 476,000, and importantly, found no cases of stroke or cauda equina syndrome (Chu 2023). This means that serious complications are extremely rare.

Based on the table, the risk of serious complications from chiropractic care is classified as very rare to extremely rare, while the risks from common medical and surgical treatments are substantially higher. However, this does not mean higher-risk interventions are inappropriate; they are often necessary and suitable for patients in specific clinical situations where the potential benefits are judged to outweigh the known risks.
Ultimately, determining the most appropriate treatment pathway often requires collaborative, "joined-up" care. This involves different healthcare professionals working together in to assess a patient's individual needs and create a tailored plan that can safely include different types of intervention at the right time.
How Chiropractors Prioritise Your Safety
UK chiropractors are highly trained and regulated professionals. They undergo a minimum of four years of university-level education, with specialised training in diagnosing and managing spinal disorders, neuro-musculoskeletal conditions, and clinical imaging, alongside manual therapy techniques.
A key aspect of their training focuses on clinical reasoning, identifying ‘red flags’ (signs of serious underlying conditions), and thorough risk assessment. This means your chiropractor is specifically trained to recognise when chiropractic care is appropriate for you and, crucially, when it's necessary to refer you to another healthcare provider, such as your GP. This commitment ensures that you receive effective and safe care within clearly defined clinical boundaries.
The Importance of Early MSK Care
When access to early, appropriate care is delayed, patients often face a greater likelihood of long-term medication use, with nearly 47% of people with long-term MSK conditions in England getting prescribed five or more medications (Versus Arthritis, 2021; Rheumatology Advisor, 2021). There's also a risk that some individuals are referred for injections or surgery without first exploring lower-risk options (Rooshenas et al., 2022), (NHS England, 2022 – Referral Optimisation Guidance). This is a concern because research indicates that opportunities for recovery can be missed when there are prolonged waiting times for MSK treatment, potentially leading to increased pain and reduced quality of life (getUBetter, 2023).
This is where chiropractic care can play a vital role. By providing assessment, manual therapy, exercise guidance, and self-management support in community settings, chiropractors can enhance service capacity and offer patients timely, effective care options. The British Chiropractic Association supports an approach that prioritises early access to high-quality, patient-centred care for musculoskeletal conditions, viewing chiropractic care as a safe, effective, and often underused component of this solution.
Contributing to MSK Waiting List Recovery
Recent research by the British Chiropractic Association and the York Health Economics Consortium (YHEC) indicated that integrating chiropractors into NHS MSK pathways could significantly alleviate current service pressures. Our analysis estimated that over 100,000 additional MSK appointments could be delivered annually through chiropractic care, effectively reducing waiting times and improving patient access to treatment.
Beyond clinical benefits, the report highlighted substantial economic advantages. By enabling patients to return to work more promptly, the UK economy could see productivity gains of approximately £400 million per year, amounting to over £1.5 billion across the current parliamentary term (YHEC, 2024).
Chiropractors, as regulated healthcare professionals with expertise in MSK conditions, are well-positioned to support the NHS in addressing the backlog of cases. Their inclusion in community-based care models and multidisciplinary teams can enhance service capacity, reduce reliance on pharmacological interventions, and provide patients with timely, effective care options.
Improving access to timely, conservative MSK care is not only a matter of system efficiency - it is about addressing patients’ unmet need with compassion and pragmatism. Chiropractors, as part of the MSK workforce, can provide assessment, manual therapy, exercise guidance and self-management support in community settings. Their inclusion in multidisciplinary teams would allow for earlier intervention, more personalised care, and improved patient outcomes.

Many patients desire more meaningful non-surgical options for their spinal pain, a need often born from frustrating NHS encounters where they feel their concerns are dismissed. As one patient recalled being told by their healthcare provider: "they just said it’s just down to wear and tear basically and you’ve got to live with it." (Pearson 2016) This sense of dismissal is directly at odds with the logical, stepped-care approach that patients wish to follow. This ideal philosophy was clearly articulated by another patient discussing his experience with an NHS service: “You’ve got to try something... it’s easier to resolve it in the simplest ways rather than go into the extreme ways, because maybe you don’t need to go to the extreme...” (Singh 2018) These sentiments reveal the gap between the care some patients receive and the care they seek. They highlight a clear need for early, supportive options that can help patients recover without an unnecessary reliance on medication, injections, or surgery.
The General Chiropractic Council (GCC) further reinforces patient confidence. As the regulatory body for chiropractors in the UK, the GCC ensures that all practicing chiropractors meet high standards of education, conduct, and practice, protecting patients through strict adherence to a Code of Conduct and ongoing professional development requirements. The GCC's 2023 Patient Satisfaction and Experience Report confirms that "Overwhelmingly, chiropractic patients report high levels of satisfaction and positive experiences with their care" (GCC UK, 2023).
